At Health Partners LLC, we know how different needs can be for all individuals. That is why our providers have created a number of options to suit your individual preferences.
You may become a new patient through our "Traditional Model" or through our Health Consulting Service.
Functional Healthcare
We offer both in-person and virtual appointments. Our preference is to see new patients in person to get to know you on a direct level, but we recognize this is not possible in all cases. We offer remote/telehealth appointments for those who are unable to leave home or live a significant distance away from our Northeast Florida office.
Appointments are charged by time and based on our current hourly rate.
Minimum appointment charge is 30 minutes.
Additional time in excess of 30 minutes is charged in 15 minute increments.
Acute appointments: less than 15 minutes and covering one acute symptom only (examples include UTI, respiratory virus, rash, etc.) are charged at an acute rate.
On-site care: we offer a limited number of on-site patient visits to individuals and small businesses on a case-by-case basis. Note that travel fees will be assessed and patients advised of cost prior to booking appointment. Travel fee will be collected prior to appointment time.
One time setup fee is added to all initial patient appointments.
DOCUMENT REVIEW/ADMINISTRATIVE
Additional charges may be applied for review of previous records based on number of pages/documents provided.
Additional charges may apply for insurance forms, communications, letters, as well as other items requiring additional provider/administrative time.

Offered for those individuals who prefer to order diagnostic testing/evaluation without input from our practice.
Follow up review/discussion of diagnostics is available at our hourly rate (1/2 hour minimum) for those desiring a professional review.
Cognitive Services
Lynn has been certified in The Bredesen Protocol for Cognitive Decline for multiple years, and is pleased to offer the following “one-time” packages for those individuals interested in a one-time evaluation of risk and also for those experiencing early symptoms. Note that both Precode and Recode packages include multiple diagnostics as well as a follow up appointment (up to 45 minutes) to review diagnostics and risk.
Please be aware that most individuals experiencing symptoms prefer to become patients to allow for a more comprehensive evaluation and physical examination.
Precode: for those with no current cognitive symptoms but interested in understanding key risk factors related to cognitive decline. Addressing risk factors early can prevent onset of cognitive decline symptoms.
Long Covid:
Functional Medicine provides an ideal approach to working with patients suffering from chronic symptoms of Covid infection. Covid can and often does affect multiple body systems, and the long term effects can persist for months and possibly much longer. Rebuilding immune function, ensuring a properly functioning GI system (the bulk of your immune function originates here), and correcting deficiencies are all important components of a return to health.
Our approach involves a detailed evaluation of current health, diagnostics as indicated, and an individualized plan to improve overall health. Priced at our hourly rate plus diagnostics as indicated.

Health Consulting Services
For more than 6 years, our practice has employed a Functional Medicine “systems” approach to helping patients with complex, multi-system health conditions. These patients often come to our practice frustrated because they are dealing with multiple conditions and multiple specialty providers, but aren’t experiencing improvements in health. Time and again, I hear stories from patients who inherently suspect there is “something more going on” and just can’t seem to find anyone to take the time to listen and fully evaluate their health from a “systems” approach.
Have you been struggling with your health for years? Have you seen numerous specialists and become frustrated by the lack of a comprehensive review of your health? Do you often wish there was someone available to review your case and provide suggestions about approaches? All too often, this describes the reason our patients come to Health Partners LLC.
Health Partners LLC patients experience the benefits of a Functional Medicine based “systems” approach to health- it is the core of how we approach patient health. However, I recognize that not everyone wants to establish a regular relationship with our practice, but instead would prefer a “one time” thorough evaluation of health. My goal is to help patients understand and address their health to enable improvement and healing.
Health Partners LLC is pleased to offer Health Consulting services to help you understand the complex relationships between your new and chronic medical conditions. The purpose of this service is to help you understand your personal health picture from a “systems” approach rather than an individual organ or condition. Lynn will spend the time necessary to conduct a thorough review of your health, diagnostics, medications, supplements, and treatments. She will then provide the education and support you need to interact with your specialty providers. The goal of this service is to help you receive the healthcare support needed to address your unique and complex health issues. Health Consulting service is now available to a limited number of patients on a case-by-case basis.
How does Lynn’s background enable her to provide this unique approach?
Lynn is a Family Nurse Practitioner with Advance Practice training in the Functional Medicine approach to health. Functional Medicine takes a “systems-based” approach to evaluate and understand all components of health, rather than simply looking at symptoms in relation to a specific organ. 2022 marks her 6th year working with complex patients through this approach. Prior to practicing medicine, Lynn spent years in scientific research studying complex human diseases and conditions. She also functioned as a key decision maker on funding for University-based research projects through her role as Director of Research and Development within the New Jersey Commission on Science and Technology. She credits her ability to think critically and deeply with her varied experiences, and believes strongly in the patient-provider relationship.
FAQ
How much will Health Consulting cost?
Lynn charges an hourly rate. Your cost will depend upon the complexity of your health conditions. Most patients will require between 2 and 6 hours total, including the report.
What is included in this service?
1) Initial phone meeting with Lynn to overview your concerns and health conditions (up to 30 minutes- additional time at patient’s discretion). During this meeting, Lynn will outline the records she will need for review of your case.
2) Evaluation: based upon initial discussion and records received, Lynn will conduct a thorough review of your health status and areas of concern, noting any diagnostics out of range or trending out of range. (30 minutes to several hours based upon the volume and complexity of records received)
3) Evaluation of medications and supplements
4) Report: Lynn will prepare a report summarizing her findings. As part of this report, Lynn will make recommendations related to next steps, including additional diagnostics/imaging, recommended medications/supplements, and additional specialty providers to further address your unique health conditions. Please note that Lynn will not prescribe medications, or provide insurance based diagnostic/ imaging orders as part of this evaluation- this service can only be done with patients of record. (30 minutes to several hours based upon the volume and complexity of the review and extent of findings)
5) Optional final meeting with Lynn to review your report. (30 minutes to one hour based upon extent of findings)
Can I use my insurance for this service?
Simply put, the answer is generally no- charges are considered “out of pocket”. I do not accept insurance, given that insurance coverage changes continually and is very much plan-based. Your costs may be covered by your health savings account. We suggest you contact your insurance company with any questions here.
If you have health insurance coverage, we will work with you to order any indicated diagnostics through your insurance where possible. Note: Some diagnostics are not covered by insurance, and we will always communicate pricing and obtain your consent before ordering.
*Please note it is your responsibility to determine insurance coverage and any copays or diagnostic charges are your responsibility as coverage can be quite variable. Private pay diagnostic prices depend on individual recommendations. Please note that private pay pricing is communicated to the patient and diagnostics are never ordered without patient approval.
We recognize that health care can be expensive and promise to always try to find reasonably priced diagnostics and treatment.
Supplement purchases are not included in the appointment cost.
