Who We Are

We are a team of healthcare professionals who believe health is both a personal responsibility and a personal choice. We further believe that optimal health can only be attained when the body functions properly. As such, we practice from a functional medicine perspective. Functional medicine serves as a “bridge” between standard pharmaceutical-based medicine and holistic therapies, focusing on restoring the body’s ability to function in the manner it was designed.

What We Offer

We offer both in-person and virtual appointments. Our preference is to see new patients in person to get to know you on a direct level, but we recognize this is not possible in all cases.  We offer remote/telehealth appointments for those who are unable to leave home or live a significant distance away from our Northeast Florida office.

Our Philosophy

The functional medicine model is an individualized, patient-centered, science-based approach that empowers patients and practitioners to work together to address the underlying causes of disease and promote optimal wellness. It requires a detailed understanding of each patient’s genetic, biochemical, and lifestyle factors and leverages that data to direct personalized treatment plans that lead to improved patient outcomes.

Lynn-Kettell Slifer, APRN, MSN, RN-BSN

Lynn Kettell-Slifer is a Family Nurse Practitioner licensed in several states, including her “home base” of sunny Florida. Her educational background includes multiple graduate and undergraduate science, technology, and Nursing degrees, completion of the requirements for Autonomous Practice in the state of Florida, Advanced Practice training in Functional Medicine, Certification in The Bredesen Protocol for Cognitive Decline, Reiki training, and countless Continuing Education credits covering a multitude of areas within both allopathic and Functional medicine. Her focus areas include tick-borne illness, Mold/mycotoxin illness, autoimmune conditions, hormones, GI, and cognitive decline.

Lynn’s career background includes years in research and development, directing R&D and Venture Development for the NJ Commission on Science and Technology, consulting on health related issues, volunteering and working in underserved clinics/medical missions, and opening/managing a series of primary care clinics.

She has written numerous health-related articles and presented at multiple organizations and venues.

Lynn holds memberships in multiple organizations including The American Association of Nurse Practitioners, The Institute for Functional Medicine, The Association for Advancement of Restorative Medicine, and Realm of Caring.

Lynn credits her strong faith in Jesus for continued guidance, and in her restorative time enjoys biking/hiking, travel, time with family, and working with her rescue dogs.

In 2016, Lynn decided to open her own practice to enable her to address the shortcomings of insurance- based medicine. Her goal was to return to the true roots of health care, spend the time needed to understand and treat each patient as an individual, and use integrative modalities of treatment known through evidence-based research to improve symptoms and restore health. Health Partners LLC was launched, and continues to thrive and grow. Lynn is passionate about treating each patient as an individual- this approach gives her the ability to truly understand and treat the often overlooked causes of declines in health.
